Purpose The Substance Abuse Counselor provides community-based case management and psychosocial rehabilitation in collaboration with the consumer and recovery team members. The Substance Abuse Counselor guides and provides services to Community Treatment consumers that are consistent with the Integrated Dual Disorder Treatment (IDDT) model and work to enhance fidelity to this model of Dual Disorder Treatment. Education and Experience Requirement Minimum Education \u2022 Current Wisconsin certification as a Clinical Substance Abuse Counselor or Substance Abuse Counselor Minimum Work Experience One to three years experience Required Skills \u2022 Ability to perform an assessment, create a treatment plan, and make recommendations based on findings \u2022 Ability to collaborate with community partners \u2022 Strong verbal and written communication skills \u2022 Ability to multi-task \u2022 Ability to perform under pressure Driving Requirements: \u2022 Valid Wisconsin driver's license and appropriate auto insurance coverage as required by North Central Health Care. \u2022 Must have the use of a personal vehicle. Preferred: \u2022 Three to five years' of experience \u2022 Knowledge of motivational interviewing techniques and\/or experience with motivational interviewing Any combination of education and experience that provides equivalent knowledge, skills and abilities may be considered. Essential Duties and Responsibilities Provide consultation, direction, education and clinical guidance to Community Treatment program staff related to substance use disorders.Conduct person-centered assessment including assessment of consumer strengths and needs.Formulate person-centered recovery plan with clear documentation of goals, objectives, strengths, barriers and interventions.May provide direct psychosocial rehabilitation services including communication and interpersonal skills training, psychoeducation, recovery education\/illness management and group facilitation. SPS 160.02(10)]: screening, intake, orientation, assessment, treatment planning, counseling, case management, crisis intervention, client education, referral, reports and record keeping, consultation with other professionals regarding patient treatment and services \u2022 Adheres to all regulations in State of Wisconsin Chapter DHS 75 \u2022 Completes two hours of clinical supervision for every 40 hours of work performed by a substance abuse Counselor [ref. SPS 162.01(1)(a)] \u2022 Maintains current Wisconsin certification as a Clinical Substance Abuse Counselor, Substance Abuse Counselor, or Substance Abuse Counselor In Training as required by their position \u2022 Provides and coordinates services that build on strengths, choices, preferences and stage of change \u2022 Completes documentation in accordance with CCS, CSP, CRS, COP and Medicaid standards \u2022 Works effectively and collaboratively with colleagues, consumers and community partners \u2022 Participates in the Quality Improvement goals of the program and agency \u2022 Has the personal aptitude, interpersonal skills training and life experience demonstrating an ability to provide positive and effective services for patients\/clients\/consumers with mental health and\/or substance disorders \u2022 Works within a recovery model that promotes hope, healing and empowerment for patients\/clients\/consumers \u2022 Establishes rapport, gathers data, determines readiness for treatment and change, applies accepted criteria for diagnosis of substance use disorders \u2022 Screens for co-occurring mental health issues \u2022 Analyzes and interprets data to determine treatment recommendations and priorities \u2022 Formulates mutually agreed-upon measurable treatment goals and objectives with individuals served Job Specific Competencies Assesses consumers substance use, strengths and treatment needsEstablishes a diagnosisCollaborates with recovery team to provide person-centered care from assessment through service delivery Core Value Standards of Behavior It is expected that all employees will demonstrate behaviors that support excellence as defined by North Central Health Care's Core Value Standards of Behavior.
